Immunization Requirements
The Ministry of Education requires that every student (ages 4 to 17) has a complete and up-to-date immunization record on file with their local health unit to attend school. Contact Niagara Region Public Health to check that your child’s immunization records are up-to-date.
New to Ontario?
If your child goes to school in Ontario, you must report their immunization record to Public Health. Public Health will review your child’s record to make sure it is up to date and meets Ontario’s vaccine requirements for school attendance.
Vaccine schedules are different across Canada and around the world. Your child may be up to date in your home country, province or territory, but not in Ontario.
Report your child’s immunization record
If your child has an Ontario health card, follow these reporting instructions.
If your child does not have an Ontario health card, you can send your child’s immunization record to Public Health by email at vpd@niagararegion.ca. Please include:
- A clear photo or scan of the immunization record
- Your child’s name
- Your child’s date of birth
If you do not have an immunization record, call Public Health.
Note: Email is not secure. Please include only limited personal health information. Other reporting options are available.
What happens next?
After you report your child’s record, Public Health will review it. You may be contacted if more information is needed or if your child requires additional vaccines.
If your child needs vaccines and you do not have a family doctor, you can book an appointment with Public Health. You do not need a health card.
Important
You may receive a notice from Public Health if we do not have an up-to-date immunization record or valid exemption on file.

You will hear a voice recording. Press 1.
You will hear a second voice recording. Enter 7425.
You will hear a third voice recording. You will then be able to select one of these options:
- Press 2 if you need a language interpreter
- If you need a language interpreter, let us know the language you speak or the country you are from and we will add an interpreter to the call
- Press 3 to book a vaccine appointment
- Press 4 to report an immunization
- Press 5 if you require ICON support
- Stay on the line for all other vaccine inquiries (for example, to speak with Public Health to make sure your child has all the vaccines to attend school in Ontario)
