Please note that the DSBN Education Centre and Record Centre offices will be closed the last two weeks of July for the annual summer shutdown.
For requests to be processed before the shutdown, the submission deadline is Thursday, July 10, 2025.
Any record requests received via email during the summer shutdown, will be addressed upon staff's return on a first received, first completed basis. Responses may take longer than normal depending on the volume of requests received.
The Transcript Office will reopen after the shutdown on Tuesday, August 5, 2025.

This webpage is to assist requesters on the process to follow in accessing a variety of documents.
- Secondary School Transcripts
- Replacement Diplomas
- Proof of Education in English Letters
- Other Record Requests:
- PERSONAL RECORDS (i.e. requests for information about you or your minor child(ren))
Initially, requests for personal records should be directed to the current school
attended (for current students) or the last school attended (for former students).
Most personal record requests will be provided as an informal routine
disclosure
- OTHER’S PERSONAL INFORMATION BY AUTHORIZED PARTY (accompanied with written consent)
Record requests by authorized parties (e.g., legal counsel, Ontario Children’s
Lawyer etc.) should be directed to FOIRequests@dsbn.org and managed as a
Freedom of Information request
- CORRECTION TO OWN PERSONAL INFORMATION
Initially correction requests should be directed to the current school attended
(for current students) or the last school attended (for former students)
- GENERAL RECORDS (i.e. requests for general information not related to you)
Initially, requests for general records should be directed to the DSBN
department best associated with the information being requested to determine
whether the enquiry can be resolved through an informal process or other
internal process

Parents requesting Canada Revenue Agency (CRA) letters must make a written request to the school currently attended by their child(ren). The school will provide registration and residency information in a letter for CRA purposes. If the student is no longer attending a DSBN school, the parent must contact the Area Superintendent’s office for the applicable DSBN school where the child last attended to prepare the CRA letter.
